Insurance Coverage that meet or even exceed the J1 Requirements
Dear Sir/Madam, I am an international (Iranian) Master student at Politecnico di Milano,Italy, and I plan to come to United States as a J-1 visiting scholar for 9 months starting from September 21, 2015. The destination institute, UCLA, needs a specific kind of insurance with minimum levels of coverage that meet or even exceed the J-1 requirements as follows:
No overall maximum dollar limit for medical coverage
Annual deductible of no more than $500
Coinsurance that does not exceed 25%
Medical evacuation coverage must be at least $50,000
Repatriation coverage must be at least $25,000
Pre-existing conditions must be covered with no waiting period or exclusions or dollar limits
Maternity must be a covered benefit
Mental Health and Substance Abuse services must be a covered benefit
Preventive Care & Wellness Services must be covered with no cost (includes immunizations and screening tests)
Prescription drugs must be a covered benefit
Carrier must be at least A- rated or backed by the full faith and credit of the Exchange Visitor's government
And finally after my subscription, l need a proof of my insurance coverage by providing a copy of my insurance policy or schedule of benefits written in English to present to the destination university and USA embassy for J-1 visa application. Could you please provide some information regarding the appropriate insurance plan that I should buy and the procedure of making a contract with these requirements, and maybe the cost of it for 9 months or one year.

Below are the plans that satisfy most of the specified requirements. The plans do have a waiting period for covering pre existing conditions. If it is ok with you and University, then you can go ahead with the purchase of these plans.
